Forms
All forms must be filled out and returned to SAS by emailing pssdinfo@peralta.edu. If at all possible, please fill the forms out electronically.

Mandatory forms:
- Application for Services: PDF fillable application
- Authorization to Exchange Information: PDF fillable Authorization to exchange information || Word fillable authorization to exchange information
- Voter preference form. (“Would you like to register to vote?”)

Other forms:
- Verification of Disability: PDF fillable disability verification || Word fillable disability verification
- Fill out the Student Section, then send to your doctor/therapist/other licensed professional to fill out the Professional Section.
Documentation should be emailed to pssdinfo@peralta.edu.
Please attach documentation directly to the email. Documentation sent through Google Drive is often inaccessible due to incompatibility with Microsoft Outlook.
Until we get Docusign up and running, please include a statement confirming that you are applying to our program, and agree to the Student Responsibilities and Agreement in the application.
